Buckeye Health Plan Launches Buckeye Community Connect Online Database

Buckeye Health Plan has launched Buckeye Community Connect, a new searchable database of services and resources to help Ohioans in need improve their overall health and wellness. The free tool, found at communityconnect.buckeyehealthplan.com, provides access to an ever-growing database of more than 40,000 community partners statewide.

Buckeye developed the tool to help address many of the Social Determinants of Health (SDOH) negatively impacting Ohioans in need including medical care, food, job training, transportation, legal assistance and more. It also enables healthcare providers, social workers and care managers to quickly and easily find local resources available to help their patients.

“Good health goes beyond the walls of clinical care. Social Determinants of Health such as education, employment and access to food and housing can greatly impact overall health and well-being,” said Steve Province, President and CEO of Buckeye Health Plan. “By centralizing these resources and services, Buckeye can bring valuable local resources directly to the people who need them most.”

SDOH factors drive as much as 80 percent of health outcomes. U.S. healthcare costs approached nearly $3.5 trillion in 2017 and costs are expected to climb to $5.7 trillion by 2026, representing nearly 20 percent of the economy. Given these factors, services like Buckeye Community Connect, are critical to the health and well-being of all, and can be especially impactful to reducing barriers for vulnerable populations.

“To truly improve community health, we must consider the everyday challenges that can have a negative impact on life, health and wellness. Buckeye Community Connect equips care providers, community workers and anyone in need with tools and resources to address each challenge,” Province said.
