Required Training

Special Needs Plan Model of Care Self-Study Program

The Centers for Medicare & Medicaid Services (CMS) require health plans to provide annual education and training regarding our Special Needs Plan (SNP) Model of Care to providers who treat our SNP members. This applies to our Dual Eligible Special Needs Plan (D-SNP) members, who are eligible for both Medicare and Medicaid, and our Chronic Condition Special Needs Plan (C-SNP) members.

As stated in our provider manual, all providers who treat our SNP members, regardless of network participation status, must complete Model of Care (MOC) training annually by Dec. 31.

The training is designed to help you better understand our approach to the delivery of care for SNP members.

The SNP MOC training is available below for download and self-study. Please be sure to complete your Attestation when it is completed.

Buckeye Health Plan encourages our providers to engage in Cultural Humility trainings and education to promote positive interaction with diverse cultures.

Improving and practicing cultural awareness, humility and mindfulness can help you better understand and respond to the needs of patients while maintaining a professional level of respect, objectivity, and identity. 

If you do not have an appropriate training available to you, please complete Buckeye Health Plan's Cultural Competency Training at the link below.

For Large Groups

After reviewing the training at the PDF link above, download and complete the Provider Attestation Spreadsheet. Once all members of the group are added to the spreadsheet, please submit the spreadsheet to the Health Equity Project Manager, Monique Webster via email. Your completed spreadsheet will be confidential and uploaded to a secure site.

Effective 12/31/2018, the Ohio Department of Medicaid (ODM) and Buckeye Health Plan (Buckeye) will implement new signature requirement for MyCare waiver services. On and after that date, waiver service providers will be required to sign the individual’s person-centered service plan (service plan). This change meets the Centers for Medicare and Medicaid Services’ (CMS) 42 CFR 441.301 rule requiring the service plan “be finalized and agreed to, with the informed consent of the individual, in writing, and by all people and providers responsible for its implementation.”

The provider’s signature confirms that the provider acknowledges and agrees to provide the waiver service, as authorized in the waiver service plan. The signed service plan must be completed and returned to Buckeye to ensure timely payment for services. Requirements will vary depending upon the services provided.
