CBP: Controlling Blood Pressure

We want to reduce chart chases and staff interruption for our provider offices by equipping you with clear, practical insights for the Controlling Blood Pressure (CBP) HEDIS Measure — helping align evidence-based hypertension care with documentation standards that support quality performance and population health goals.

Below are some highlights to remind you what to capture when documenting your patient’s medical chart. You can find the complete measure in our HEDIS 2026 Booklet on our website.

The information below may be helpful to reduce chart chases and staff interruptions for your office. Accurate measurement, complete documentation, and correct CPT II coding are essential to meeting CBP quality requirements.

  1. Did you know that to get credit for the CBP measure, your patient must have a documented blood pressure reading below 140/90?
  2. If you check your patient’s blood pressure upon arrival in your office and the reading is above parameters, please document a second reading prior to your patient leaving your office. If the blood pressure remains outside the parameters, does a medication adjustment need to occur?
  3. Representative blood pressure reading documentation:
    • Use the most recent BP recorded on or after the second hypertension diagnosis.
      • If multiple readings are taken on the same date, use the lowest systolic and lowest diastolic values.
  4. You are non-compliant if:
    - BP ≥ 140/90 mm Hg
    - A missing BP
    - Or, missing systolic or diastolic value
  5. Coding – You can bill with the following codes:
    Systolic < 130 --- 3074F           Diastolic < 80 --- 3078F
    Systolic 130-139 --- 3075F      Diastolic 80-89 --- 3079F
    Systolic > 140 --- 3077F           Diastolic > 90 --- 3080F
