Training Attestation

Buckeye Health Plan provides several self-led provider trainings. These are annual trainings that is offered to every provider and are available 24/7 on the Provider Training Page. After completion of the training, providers will then need to complete the attestation.

If you are required to complete the Cultural Competency Attestation for multiple individuals, please note that you can streamline the process by downloading an excel file.
