Welcome to the Specialty Medication Prior Authorization page. Effective 1/1/22, we have streamlined our procedure to find a Prior Authorization Form for a specialty medication. Rather than the previous Search Function, we now have one General Specialty Medication PA Form for use across all specialty medications. Please note, there is also a new fax# on the form.
