How to Join

First, if you have questions about Buckeye Health Plan, please call our Member Services Department at 1-866-246-4358 (TDD/TTY: 1-800-750-0750) and we will be glad to assist.

From there, if you’d like to choose Buckeye as your health plan, here are the things you need to do:

• Visit www.benefits.ohio.gov to check eligibility for Medicaid and apply online (this is the fastest way)
• Or, call the Ohio Medicaid Consumer Hotline at 1-800-324-8680 and request an application which can be submitted by mail or you may apply in person at your local CDJFS (County Department of Jobs and Family Services) office. Go here to locate an office near you.

You may also call the Medicaid hotline at 1-800-324-8680 to get help with applying for Medicaid and selecting Buckeye Health Plan.
