Preparing for your firstappointment:

We will work with you to create a treatment plan specific to your needs. Your first appointment with a behavioral health specialist follows three steps:

  • Screening: Answer questions about your current
situation and challenges to determine the appropriate
    services for you
  • Intake: This includes getting information such as
insurance card, ID (driver’s license or state ID) and list
of medications and healthcare providers. If you are
a guardian or have a guardian, it’s important to bring
proof of this relationship to your first appointment
  • Assessment: Your specialist will ask for more in-depth
information about your symptoms and your personal
history to establish diagnoses and a treatment plan
