Leaving the hospital is just the first step. Buckeye is here to support your healing every step of the way. If you’ve just been in the hospital for mental health care, it’s important to keep getting support. Seeing a provider and going to follow-up visits can help you stay on the path to positive mental health.

What to Do After You Leave the Hospital

Within 7 days:

Make an appointment with a mental health provider. They can help with your medicine, create a treatment plan and connect you to ongoing support.

*Follow up with your primary care provider to schedule an appointment if you are not able to see a mental health provider within 7 days of discharge.

Within 30 days:

Keep going to counseling and follow-up visits. Buckeye can help you get support for:

  • Peer support and group programs
  • Housing, job help and other community services

Feeling unsure about what to expect? These simple steps can help you feel ready for your visit.

  • Bring your hospital discharge papers and medicine list.
  • Write down your questions ahead of time.
  • Bring a friend or family member for support, if it’s helpful.
  • Talk to your provider about any changes in how you feel.
